Personnel Leave Request Links

As of 2024-2025 school year, all departments will be accessing the Personnel Leave request via Informed K12. Below you will find links to your specific employee group along with instructions.

How to complete the online Personnel Leave Request

  1. Click on the form link/scan QR code to initiate the Personnel Leave Request Form
  2. Complete the fields requested (employees should use their district provided email)
  3. Click Go to form
    • Complete all required fields
      1. Employee # is the six digit ID number provided to employees (number can be found on the back of employee badge).
      2. Worksite is the employee’s primary location
      3. Payroll period leave dates entered should be within this date range. Complete an additional form to dates outside of the date range.
    • Enter legal first and last name
    • Enter dates and hours of leave
      1. Date and total amount of hours must be completed for each date entered
      2. Review “Total # Hours” to ensure the request is correct
      3. Review dates and payroll period. Dates must be within the payroll period date range (Additional dates outside of the date range will require an additional form to be completed.)
    • Select type of leave
      1. Only one (1) leave type may be used per form
    • Reason must be entered for Personal Necessity, Bereavement or FMLA/CFRA
      1. Personal Necessity - enter the reason (see allowable reasons on form)
      2. Bereavement - enter the relation to the deceased (see list of qualifying relations on form)
      3. FMLA/CFRA must have pre approval from HR (employee may enter FMLA or CFRA in the reason field)
    • Review form **Review dates to be sure they are within the specified payroll period**
    • Submit form
    • Choose timekeeper from drop list
    • A confirmation email will be sent to the email provided
